Why Dana Air Crash’s Final Report Is Delayed – AIB

The Commissioner of Accident Investigation Bureau, AIB, Dr. Felix Abali has stated one of the reasons  why Dana Air accident final report was delayed, and that the flight recorders were sent abroad for downloading and analysis since AIB had no laboratory where it could analyse accident data as at time the plane crashed

Dana Air aircraft crashed on June 3, 2012 killing 160 people both on board and on the ground.

Abali, who said this, while briefing Aviation correspondent in Lagos, explained that Dana Air accident had both local and international inputs, which unfortunately slowed down investigation by the bureau.

He however stated that since the unfortunate accident, safety had been enhanced in the country’s aviation industry, as recommendations of the agency were complied with and implemented.

The AIB boss noted further that while analyzing, one of the recorders was found to be damaged by over exposure to fire and  that the accident and incident investigation body also had challenge with getting pathology reports such as the Deoxyribonucleic Acid, DNA, and toxicology, which had to be done abroad.

According to him, “There is no DNA laboratory in Nigeria and there was a need to sign Memorandum of Understanding (MoU), with some organisations.”

At the international level, Abali declared that the agency had to get the manufacturers of airframe and the engine involved in its investigation, adding also that it had to do engine teardown while the National Transport Safety Board, (NTSB), of the United States was actively involved.

He said that the bureau had to depend on the other parties for certain things to be done, stressing that travel logistics for accredited stakeholders were necessary and carried out.

In his words, “Simulation was carried out overseas with various organisations in attendance. Reports were needed from these parties. Statutory 60-day required for comments as demanded by the International Civil Aviation Organisation, ICAO, had to be complied with. All these contributed to the delay in the releasing of the final report by us.”

The AIB Chief Executive Officer stated that the agency had made some safety recommendations to both the regulatory body and the affected airline, which he said were complied with.

Accident investigation, Abali pointed out may also involve safety issues that may not be connected with the accident under investigation, assuring that AIB was on the last leg of the report having concluded the investigations.
